Mortgage Change: Tips and Steps

I have been living with my partner for 4 years now, who has his own home. The mortgage was co-signed by a third party and this is also stated on the deed. Now my partner wants to get rid of this third party and have me on the mortgage. This third party agrees to this and does not need any compensation or anything like that. He has never lived there either, only co-signed because my partner could not get the mortgage otherwise. How can we change this third party to me as cheaply as possible? I have a permanent job with a fixed income, the third party has an AOW benefit.
