Sample letter for changing employment contract

Is there a sample letter (addendum to employment contract) regarding the change of contract hours, salary and function? Or do you then enter into a new contract from the set date? The employee has agreed to it and already has a permanent contract, the change has just not yet been recorded in writing?

If you are already employed by the employer and your position changes, your old employment contract will be revised and you will enter into and have to sign a new contract.
