Is there a sample visitation schedule that I can use, of what exactly it should contain and such?

There is no standard visitation schedule, What is what you and your ex want? How old are the children? have you spoken to them? etc. So based on your specific situation you can agree on a visitation arrangement with your ex. Depending on the status of the (divorce) procedure this will then be confirmed in a decision of the Judge.
