Liability as secretary of a diving association: Is a so-called death certificate or vaimer legally valid?

I am secretary of a diving club. The board is considering having members sign a form in which the undersigned declares that the club will not be held responsible for any damage and/or injury. A so-called death certificate or vaimer. Is this legally valid?
