Is it smart to bring up a dismissal by mutual consent?

I have been sick at home since May 19 due to burnout, I work in the healthcare sector and have made it clear to myself that I will have to leave this sector because it makes me sick I get little understanding from the employer and I feel pressure, they want to force me to go back to work in the same position, want to adjust the contract despite illness, and their whole attitude is clear to me that they are looking for an employment conflict I have absolutely no energy for this kind of thing and it brings me down more than it brings me up, is it smart to bring up a dismissal by mutual consent? For this I ultimately have to be reported better, but what if I use my vacation for that? This way I save myself all this hassle and I can get started via the UWV to eventually go in a different direction or is it smarter to keep this illness with the employer? thanks in advance for your response
