What are the rules regarding burnout and salary increase for the end-of-year assessment for 2020?

I became ill in the last 2 weeks of December 2020 (burn-out). At the moment (2021) I am building up hours as agreed with the company doctor. We are a performance-oriented organization and every year we receive an end-of-year assessment with a salary increase. Because I am ill, I never received my assessment and increase. Is the employer obliged to give this retroactively? I was only ill for 2 weeks in 2020.

To answer your question, you should first check what your employment conditions say about this. So the employment contract, any employment/company regulations and/or any applicable collective labor agreement. I do not see why someone who was sick for only 2 weeks in the relevant assessment year is not also entitled to a (customary) assessment and any salary increase (if this follows from the employment conditions).
