Do I need to take leave for surgery?

Good afternoon, This week, after a long screening process, I received my surgery date for my gastric reduction. My employer was not happy with this and indicated that I have to reschedule. She also indicated that it is a cosmetic surgery and that it will cost me my own leave. While I do not see this as beautifying my body, but rather to become healthy.
