Stories of personal injury due to health insurance?

While cycling on the public road in a group I ended up in the verge and had an accident there. I sustained injuries and had to undergo surgery for it. Now the health insurance is trying to recover the healthcare costs from another party and I received a questionnaire for that. This questionnaire raises several questions for me: - the health insurer asks whether it is a traffic accident or a sports accident: I don't really feel like 'playing judge' in that. Can I leave this question unanswered and limit it to a description of the accident? (Namely that I ended up in the verge on the public road while cycling in a group and had an accident there) And if I absolutely have to answer that question, what should I fill in there: sports accident and/or traffic accident? - the health insurer asks if another party was involved: can I leave this question unanswered, and – here too – leave it at the brief description of the accident that the health insurer also asks for? Otherwise I will start accusing something/someone else or pleading not guilty. While I myself want to stay out of it as much as possible. - and: if the verdict is that the blame (partly) lies with me, is there a risk that I will have to pay the healthcare costs to my health insurance myself?
