Registration of sole proprietorship summons

Want to draw up a summons following an agreement where the service is not fulfilled despite all kinds of promises. An advance has already been paid, which we are going to claim back. What information should be provided about the defendant, this is a sole proprietorship, only the trade name? Also date of birth of legal entity (hodn) and Chamber of Commerce registration number?
